Kindergarten Registration

March 31st from 8:30am - 11:00am at Hayes Elementary Media Center

You should bring your child to be screened along with with the following forms:

• Birth Certificate,
• SS Card,
• updated immunization form #3231,
• Hearing, Vision and Dental Form #3300 and
• 2 forms for proof of residency.
  
  There will be a kindergarten readiness screening for your child that day.
